Job summary

Optimal Home Care & Hospice in Loveland, CO seeks a part-time Occupational Therapist to provide skilled OT services to patients in home health settings. The role requires CO licensure in good standing, ACOTE-accredited OT education, and strong communication and interpersonal skills to collaborate with the care team and families.

Qualifications

• Graduate of an OT program accredited by ACOTE.
• Must be licensed and in good standing in Colorado.
• Ability to communicate in the language of the client and document accordingly.
• Strong written and verbal communication and interpersonal skills.

Responsibilities

• Provide occupational therapy services per AOTA standards and guidelines.
• Maintain patient safety and HIPAA compliance in home health care.
• Develop and implement OT plans of care with MD collaboration.
• Document interventions and outcomes within agency guidelines.
• Coordinate care with nursing, PT, SLP, and other team members.
