Optimal Home Care is seeking a dedicated Occupational Therapist (OT) to provide comprehensive home health services to patients across Colorado.

This full-time role requires a graduate of an ACOTE-accredited OT program, current Colorado licensure, and strong communication skills. You will develop and implement the Plan of Care, collaborate with the patient's MD and the care team, and ensure compliant documentation and patient-centered care.

Formación

• Graduate of an Occupational Therapy program accredited by ACOTE.
• Must be currently licensed and in good standing in the state of Colorado.
• Must be able to communicate in the language of the client and document client related activity as appropriate.
• Possesses adequate and effective written and verbal communication skills.
• Possess adequate and effective interpersonal skills.

Responsabilidades

• Provides OT services in compliance with AOTA standards, policies, and guidelines, applicable federal and state laws and regulations, and payer requirements related to occupational therapy practice and service delivery.
• Maintains patient safety and dignity by abiding by HIPAA and agency policies and procedures.
• Develops and implements the Plan of Care in collaboration with the client(s) and MD.
• Evaluates the client’s ability to participate in daily life with consideration of goals and needs.
• Documents outcomes and updates per regulatory requirements.
• Coordinates care with Nursing, Home Health Aide, PT, SLP, and other disciplines.
