Job summary

Amazing Care Home Health Services is seeking a licensed Occupational Therapist to provide in-home, adult-focused therapy in the Denver Metro area. You will assess needs, develop personalized treatment plans, and help clients improve independence, mobility, and quality of life.

Qualifications

• A valid Occupational Therapist license in Colorado.
• Proven experience in home health or a related field.
• Excellent communication and interpersonal skills.
• Proficiency in electronic medical record documentation.
• A flexible and adaptive approach to meet the diverse needs of your clients.

Responsibilities

• Develop and implement personalized treatment plans.
• Conduct evaluations using standardized tools and clinical expertise.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Educate patients and caregivers on home programs and adaptive equipment.
• Maintain timely and accurate clinical documentation.
• Collaborate with a multidisciplinary care team to ensure coordinated support.
