Job summary

Sage Creek Post-Acute seeks a Staff Occupational Therapist to evaluate and treat patients, develop treatment plans, and supervise COTA and aides. The OT communicates with families, physicians, and the health team, and maintains accurate documentation in medical records.

Full-time role located in Nevada with responsibilities including discharge planning and staff in-services. Licensure in the state is required, and familiarity with Casamba and Point Click Care is preferred.

Qualifications

• Bachelor's degree in Occupational Therapy; Master's/Doctorate preferred.
• Proficient with Casamba and Point Click Care.
• Licensed to practice OT in the state.

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop and obtain approval for the treatment plan.
• Treat patients per physician plan.
• Supervise COTA and aides.
• Communicate patient progress with team and families.
• Record evaluations and treatment notes per policy.
• Assist with discharge planning and caregiver training.
