Job summary

Jackson Creek Post Acute is seeking a Licensed Occupational Therapy Assistant to deliver patient care under the supervision of an Occupational Therapist. You will document daily and weekly progress, implement the treatment plan, and train Restorative Aides as part of a collaborative health team.

Proficiency with Casamba and PointClickCare is required, and you will participate in discharge planning and ongoing in-services to ensure high-quality patient outcomes in a long-term care setting.

Qualifications

• Licensed as an Occupational Therapy Assistant per state regulations.
• Proficient with Casamba and PointClickCare software.
• Ability to read and understand technical procedures.

Responsibilities

• Treat patients per Occupational Therapist's treatment plan.
• Record daily treatment notes and weekly progress notes.
• Assist with training of Restorative Aides and supervise as needed.
• Communicate patient progress, problems, and plans with supervisor and health team.
• Participate in in-services and contribute to discharge planning.
• Record evaluations and recertification documentation in compliance with state and payer requirements.
