Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our healthcare team for 2 days per week in senior living facilities. You will provide direct primary care, focused on wellness, chronic disease management, and coordinating care with other providers.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Provide direct patient care and comprehensive assessments for residents.
• Conduct routine rounds and monitoring to manage conditions.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and staff on wellness and chronic disease management.
• Maintain EMR documentation with accuracy and compliance to standards.
• Participate in quality improvement initiatives to enhance care.
