Job summary

Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities. You will provide direct patient care, promote wellness, manage chronic conditions, and coordinate with other providers.

This role is flexible 3 days per week with optional weekend rounding and travel to assigned facilities. Full-time employees receive Curana Health’s benefits package and professional support.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Master’s Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents, focusing on preventive care and chronic disease management.
• Conduct regular rounds to assess residents’ conditions, adjust treatment plans, and address any emergent health concerns.
• Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the residents' needs.
• Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses to support better health outcomes.
• Maintain accurate, timely documentation within the EMR system, ensuring compliance with regulatory standards and organizational protocols.
• Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction.
