Job summary

Oak Street Health in Albuquerque, NM is seeking a Nurse Practitioner, Advanced Practice Provider to deliver value-based primary care to adults on Medicare, in outpatient clinics. You will work with a collaborative care team on a part-time basis, providing longitudinal care and promoting health equity within our community.

The role involves conducting visits, wellness exams, care coordination, patient education, and potential home visits.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in at least one NP specialty.
• Active, non-probationary state NP license and DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership with the Care Team.
• Educating patients on their health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
