Job summary

Oak Street Health in Albuquerque, NM is seeking a part-time Nurse Practitioner to provide value-based primary care to adults on Medicare. You will collaborate with a multidisciplinary team to keep patients happy, healthy, and out of the hospital.

Minimum requirements include an MSN/DNP/NP certificate, national certification in an NP specialty, active NP license and DEA license. You will conduct visits, coordinate care, and participate in population health initiatives while serving the community.

Qualifications

MSN, DNP, or NP certificate required.
National certification in one of the listed NP specialties.
Active NP license and DEA license required.

Responsibilities

Provide exceptional primary care.
Conduct office visits for routine and acute issues.
Administer annual wellness visits and health risk assessments with thorough documentation.
Coordinate care with physicians, specialists, testing facilities and care managers.
Lead population health initiatives in coordination with the care team.
Assist with phone triage and outreach to patients.
Educate patients on health conditions, care plans and treatments.
Participate in Oak Street Health promotional activities.
Conduct home visits as needed.
Perform other duties as assigned.
