UnitedHealth Group's Optum Home & Community Care in Spokane Valley, WA is seeking a Part-time Nurse Practitioner or Physician Assistant for Senior Community Care. This role offers a flexible schedule with no on-call, weekends, and holidays.

You will assess and coordinate care for high-risk members in nursing homes, senior housing and assisted living. Primary duties include comprehensive exams, documentation, care planning, and collaboration with an interdisciplinary team.

Qualifications

• Active, unrestricted licensure in WA or ability to obtain by start date.
• ANCC/AANP or NCCPA certification appropriate to NP/PA track.
• Current DEA license or ability to obtain post-hire per state rules.
• Driver's license and access to reliable transportation.
• Ability to establish collaborative practice per state requirements.

Responsibilities

• Conduct comprehensive assessments with medical history, med reconciliation, vitals and exams.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation with ICD-10 coding.
• Communicate and collaborate with interdisciplinary care team.
• Lead advanced illness and end-of-life care planning conversations.
• Counsel patients and caregivers with education and support.
