Qualifications

• Active certification and unrestricted licensure in the state of practice.
• DEA licensure/prescriptive authority or ability to obtain post-hire where required
• Driver's license and reliable transportation

Responsibilities

• Conduct comprehensive assessments including history, vitals, medication reconciliation and physical exams.
• Coordinate care with the interdisciplinary team and document ICD-10.
• Provide counseling and education to patients and caregivers.
• Support short-term sub-acute rehabilitation episodes.
