Responsibilities

• See 16–20 patients per day, including wellness visits and acute issues.
• Manage chronic diseases and co-morbid conditions.
• Review medications and orders; coordinate with physicians, specialists, and facility staff.
• Stay in touch with families and participate in care conferences.
