Telecare is seeking a Part-Time Case Manager III to support individuals with mental illness in San Diego. Under supervision, you will provide primary case management and help clients live as independently as possible in their communities.

Shifts are part-time and on-site; starting wages range from $23.00 to $26.25 per hour. Geographic pay differentials apply based on location, with starting pay commensurate with experience.

Qualifications

• Bachelor’s degree in Social or Human Services with 1 year of experience.
• Associate’s degree may substitute for two years of experience.
• Valid driver’s license and personal vehicle insurance in your name.

Responsabilités

• Serve as primary support and case manager for individuals with mental illness.
• Assist clients to live as independently as possible in their chosen community.
• Under direct supervision, perform case management duties and meet position standards.
