Responsibilities

• Provide direct support and care to clients recovering from illness in a community-based medical respite setting.
• Monitor clients' health status and communicate changes to medical staff and case managers.
• Assist clients with activities of daily living, including medication management, hygiene, nutrition, and mobility as needed.
• Coordinate with healthcare providers, social workers, and community resources to facilitate comprehensive care plans.
• Maintain accurate documentation of client interactions, care provided, and progress toward recovery goals.
• Create a safe, clean, and welcoming environment that respects client dignity and privacy.
• Support clients in accessing housing resources, mental health services, and other social supports to promote long-term stability.
• Participate in team meetings and contribute to continuous quality improvement initiatives within the respite program.
