Job summary

Qualifications

• High school diploma or equivalent.

• Flexible schedule to include nights, weekends and holidays.

Responsibilities

• Provide direct support and care to clients recovering from illness in a community-based medical respite setting.
• Monitor clients' health status and communicate changes to medical staff and case managers.
• Assist with activities of daily living including medication management, hygiene, nutrition, and mobility.
• Coordinate with healthcare providers, social workers, and community resources to facilitate care plans.
• Maintain accurate documentation of client interactions and progress toward recovery goals.
• Create a safe, clean, welcoming environment respecting client dignity and privacy.
• Support clients in accessing housing resources, mental health services, and social supports.
