Responsibilities

• Develop relationships with residents and families with a consistent caregiver approach.
• Assess and document residents' clinical condition in the EMR.
• Update care plans as needs change.
• Supervise care associates.
• Administer medications timely and accurately.
• Participate in resident outcomes meetings.
• Respond to residents and families promptly.
• Support review of clinical indicators.
• Perform other duties as assigned by RN.
