Qualifications

• Nursing degree from an accredited college or university or graduate of an approved LPN program.
• An active LPN license to practice in the State of Georgia.
• Proficient with Microsoft Office; Excel and Word.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Conducts patient/family interviews for assessment purposes and assists with evaluation of patient conditions.
• Coordinates development of care plans with measurable goals and timely updates.
• Ensures care plans are completed in accordance with regulations and with patient preferences.
