Job summary

Four County Health & Rehab is seeking a Part-Time LPN in Georgia to support nursing center operations. You will facilitate communication across departments, assist with patient assessments and care plans, and help ensure timely, compliant care delivery.

Starting pay ranges from $27.75 to $31 per hour with shift differentials for evenings, nights, and weekends. Benefits include flex pay, PTO, vision, 401(k) with match, and referral bonuses.

Qualifications

• Nursing degree or graduate of an approved LPN program.
• Active LPN license to practice in the State of Georgia.
• Proficiency with Microsoft Office suite including Outlook, Excel and Word.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members to support the care plan process.
• Assists Director with patient/family interviews for assessment and with development of care plans.
• Ensures care plans comply with regulations and are updated in a timely manner.
