Formación

• Graduate of an approved Licensed Practical/Vocational Nursing program.
• A minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Experience with electronic medical records is preferred.

Responsabilidades

• Develop relationships with residents and families focusing on physical, spiritual, emotional, and clinical needs.
• Assess and document residents' clinical conditions and notify RN/provider as needed.
• Update care plans to reflect changes in care needs.
• Supervise care associates and administer medications as ordered.
• Participate in meetings related to resident outcomes and care.
