• Licensed Practical Nurse (LPN) with current licensure.
• Minimum 1 year experience in assisted living, home health, or long-term care preferred.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Develop relationships with residents and families, placing the resident first.
• Assess and document clinical condition in the EMR; notify RN when necessary.
• Update care plans as needs change; supervise care associates.
• Administer medications timely and accurately.
• Participate in resident care meetings and review clinical indicators.
