Job summary

The Lorenz Clinic, LLC is seeking an Licensed Practical Nurse (LPN) for a part-time, non-exempt position. The role focuses on patient care coordination within the psychiatry department, with shifts on Thursdays and Fridays (16 hours per week).

Responsibilities include assisting with medication management, serving as a liaison between providers and patients, documenting vitals, and educating patients under supervision.

Qualifications

• Licensed Practical Nurse, CMA or CNA from an accredited school.
• High School Diploma or GED.
• Current and valid BLS for Healthcare Providers.
• One year of psychiatric/outpatient Behavioral Health or Addiction/substance abuse experience.
• Clean criminal background as required by law.

Responsibilities

• Assists in coordinating patient care and medication management in an outpatient behavioral health clinic.
• Acts as liaison between psychiatric providers and patients and collaborates with the care team.
• Communicates plan of care to patient/family and care team.
• Accurately document vital signs and administer medications as ordered.
• Educates patients about medications under RN/APN supervision.
• Maintains confidentiality and adheres to safety and quality standards.
