Qualifications

• Graduate of an accredited LPN program.
• Current Minnesota LPN license in good standing.
• Preferred behavioral health experience or related care setting.

Responsibilities

• Administers medications as ordered and monitors effects.
• Obtains vital signs and assists with nursing assessments.
• Documents encounters and care coordination in the EHR.
• Coordinates referrals and care across psychiatry, counseling, and primary care.
• Participates in care team huddles and crisis response as directed.
• Supports patient education and therapeutic programming.
