Qualifications

• Part-time position with 29.5 weekly hours.
• Travel locally within licensed states as needed.

Responsibilities

• Join a national network of clinicians to perform in-home health evaluations.
• Engage with patients during 45-minute visits, review meds and medical history, and perform physical exams.
• Provide Signify Health services including diagnostic and preventive screenings where ordered.
• Conduct virtual visits when needed.
• Maintain connections with health plans and coordinate referrals as required.
