Signify Health, part of CVS Health®, seeks a Part Time Physician to perform In-Home Health Evaluations for members in their own homes. The role involves detailed health assessments, care coordination, and flexible scheduling to accommodate patient needs.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Provide In-Home Health Evaluations as part of Signify Health clinical teams.
• Engage with patients in their homes to discuss health goals and care plans.
• Bridge gaps in care by coordinating with health plans and PCPs as needed.
• Travel locally within licensed states for visits and follow-ups.
• Conduct virtual visits when required and perform diagnostic services as indicated.
