Job summary

Signify Health is seeking a part-time physician to perform in-home health evaluations for plan members. You will assess medical history, perform physical evaluations, and coordinate care to bridge gaps in access to services. Virtual visits may be required, and you’ll travel locally within licensed states as needed.

Active medical licensure is required; MDs or DOs with multi-state licenses are preferred. The role offers part-time hours and access to benefits appropriate for a part-time clinician.

Qualifications

• Active, unrestricted medical license in the coverage area(s).
• Multi-state licenses preferred.
• MD or DO degree required.

Responsibilities

• Perform in-home health evaluations during 45-minute visits.
• Bridge gaps in care and connect members with the right care.
• Conduct virtual visits as needed.
• Utilize Signify Health's clinical workflow on iPad.
• Travel locally and within licensed states as required.
