Signify Health is seeking a part-time physician to perform in-home health evaluations for health plan members across the United States. You will engage with patients in their homes, review medications and medical histories, and conduct physical assessments using an iPad workflow.

Virtual visits are available as needed. The role requires an active MD/DO license, multi-state licensure preferred, and willingness to travel locally within licensed states.

Qualifications

• Active, unrestricted MD/DO license in the applicable state(s).
• Multi-state licensure preferred.

Responsabilités

• Conduct in-home health evaluations with patient interaction.
• Review medication and medical history; perform physical evaluation.
• Coordinate with health plan to connect to appropriate care.
