Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP/PA) to perform in-home health evaluations. You will conduct 45‑minute visits, review medications and history, perform physicals, and sometimes order diagnostics based on health plan needs.

You’ll join a national clinician network, travel locally within licensed states, and may conduct virtual visits. Board-certified NP/PA in good standing is required, with multi-state licenses preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
