Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required for NP or PA roles.
• Multi-state licenses preferred for flexibility across states.

Responsabilités

• Provide in-home health evaluations for patients in their own homes.
• Engage with patients to bridge gaps in care and plan next steps.
• Use iPad and clinical workflow to perform assessments.
• Collaborate with Signify Health clinicians and health plans.
• Possess flexibility to travel locally and within licensed states.
• Conduct virtual visits as needed.
