Job summary

Signify Health, part of CVS Health, is seeking a Part-time Clinician (NP or PA) to perform in-home health evaluations for members in their own homes, helping bridge gaps in care and connect patients with the right services.

The visits last about 45 minutes and include history and physical evaluation; travel within licensed states is required and use of an iPad for documentation is expected. Active NP/PA licensure and board certification are required.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
• Able to cover primary locations: Otero County

Responsibilities

• Provide in-home health evaluations for members in their homes
• Build trusted relationships to improve health outcomes
• Travel locally within licensed states as needed; conduct virtual visits when required
• Use iPad and clinical workflow to document visits; provide Diagnostic & Preventive Services as indicated
