Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to provide In-Home Health Evaluations. You will visit members in their homes, discuss health history, perform a brief physical, and coordinate with their health plan for additional testing as needed.

Visits typically last about 45 minutes; clinicians use an iPad workflow and are not required to prescribe medications or alter existing regimens.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover the following primary locations: Herkimer/ Utica, NY and surrounding area.

Responsibilities

• Provide In-Home Health Evaluations to members in their homes.
• Engage with patients to review medications and medical history.
• Perform physical evaluations and coordinate with health plans for additional tests.
• Travel locally to serve members in licensed states as needed.
• Utilize iPad and clinical workflow; no prescribing or lab orders required.
