Qualifications

• Bilingual in Spanish and English with strong written and verbal skills.
• Minimum 2 years in community health or public health with immigrant populations.
• Bachelor's degree in Public Health, Community Health, or related field; CCHW/CAC preferred.

Responsibilities

• Conduct culturally sensitive health and basic needs assessments to identify access gaps.
• Develop personalized care/resource plans aligned with health goals and social needs.
• Coordinate and deliver the Healthy Steps program and Mental Health Initiative; connect to resources.
