Job summary

NLC Support is seeking an experienced Part-Time Support Coordinator for Somerset and Middlesex Counties in New Jersey. This role involves assessing service needs and developing individualized service plans for adult clients with developmental disabilities.

The ideal candidate will have a relevant bachelor’s degree and at least one year of experience in this field. Flexibility in work and travel is essential, with a pay range of $1,500 to $3,000 monthly, depending on experience and caseload.

Covid vaccination is required, and candidates must pass a background check.

Qualifications

• One year of proven experience working with adults with developmental disabilities.
• Must pass criminal background and CARI check.
• Covid vaccination required for this position.

Responsibilities

• Assess service needs and develop the New Jersey Individualized Service Plan (NJISP).
• Establish achievable goals and service eligibility.
• Conduct ongoing monthly monitoring of clients.
• Coordinate and facilitate team meetings.
