Job summary

Catholic Charities seeks a per diem Support Coordinator to manage coordination services for participants with developmental disabilities in New Jersey. This role involves ISP development, plan monitoring, and extensive written documentation.

The position requires a Bachelor’s degree or higher and at least 1 year of related experience, plus a NJ driver’s license and solid office skills. Travel is required across Sussex County and beyond.

Qualifications

• Bachelor’s Degree or higher and 1 year of experience with adults (21+) with developmental disabilities.
• Equivalent of a year of full-time experience including paid work, volunteer work, or family caregiving.
• Must have a New Jersey driver’s license; 40–60 WPM typing; strong computer and organizational skills.

Responsibilities

• Manage support coordination services: discovery, ISP development, plan coordination and plan monitoring.
• Communicate with supervisors, participants, families, other providers, and DHS/DDD staff.
• Maintain written documentation; cooperate with inspections and agency/Division checks.
