NLC Support is looking for an experienced Support Coordinator in Burlington County, New Jersey. This part-time role involves working with adults with intellectual and developmental disabilities. The successful candidate will assess needs, develop Individualized Service Plans, and monitor progress. A bachelor's degree in a relevant field and one year of experience are required. The position offers flexible work-from-home options with a monthly pay range of $1,500 – $3,000, based on experience and caseload.

Qualifications

• One year of proven experience working with adults with developmental disabilities.
• Excellent written and verbal communication skills.
• Ability to work independently and meet deadlines.
• Valid driver's license and reliable vehicle required.
• Covid vaccination required by the state of New Jersey.

Responsibilities

• Assess service needs and develop the NJ Individualized Service Plan (NJISP).
• Establish achievable goals and service outcomes.
• Conduct ongoing monthly monitoring of clients.
• Coordinate team meetings off-site at residential or day programs.
