Job summary

Nlc-Support is seeking an Experienced Support Coordinator (Part-Time) to serve Bergen/Passaic Counties. The ideal candidate will assess client needs, create NJISP plans, and coordinate services for adults with developmental disabilities.

Requires one year of relevant experience, a bachelor’s degree, a car, and willingness to travel; vaccination and background checks are mandatory. Flexible schedule and potential for advancement offered.

Qualifications

• One year of proven experience working with adults with developmental disabilities.
• Initiative, ability to work independently, and meet deadlines.
• Excellent written and verbal communication; reliable car; availability for meetings during the day.
• Hybrid position requires field visits; travel within Bergen/Passaic counties.
• New Jersey vaccination is required; background and CARI checks needed.

Responsibilities

• Assess service needs and develop the New Jersey ISP (NJISP) for access to services.
• Establish achievable goals, outcomes, and service eligibility.
• Develop a Person-Centered Planning Tool (PCPT) to guide the planning process and NJISP.
• Ongoing monthly monitoring of clients.
• Coordinate and facilitate team meetings off-site at Residential and/or Day Programs to identify service needs.
