Job summary

LilyCare Hospice is seeking a part-time Hospice Social Worker to join our interdisciplinary team in northern New Jersey. This role offers a path to full-time and involves working out of Florham Park, NJ, with a focus on shaping end-of-life care for patients and families.

The position requires an MSW and active NJ LSW/LCSW license, plus at least one year of healthcare social work experience. Travel and a strong care-centered approach are valued in our community-based program.

Qualifications

• MSW from a program accredited by the CSWE.
• Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners.
• At least 1 year of social work experience in a healthcare setting; hospice, home health, or geriatric experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Serve as the Hospice Social Worker for patients and families on your caseload — assess psychosocial needs and build the social work portion of the individualized plan of care.
• Provide counseling and support to patients, families, and caregivers, including anticipatory grief and bereavement follow-up.
• Evaluate insurance coverage and financial need, and connect families to community resources and benefits programs.
• Participate in IDG meetings and keep the interdisciplinary team current on psychosocial changes.
• Complete timely, accurate EMR documentation and support volunteer and staff orientation.
