LilyCare Hospice in New Jersey is seeking a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. The role is based in Florham Park, with potential to move to full-time.

You will assess psychosocial needs, provide counseling to patients and families, review insurance coverage, and participate in IDG meetings. The position requires an MSW and NJ LSW/LCSW licensure, plus a valid driver's license.

Qualifications

• MSW (CSWE-accredited) required.
• Current NJ LSW or LCSW license required.
• Minimum 1 year of social work experience in a healthcare setting preferred.

Responsabilités

• Serve as Hospice Social Worker for patients and families on caseload; assess psychosocial needs and contribute to care plan.
• Provide counseling and support to patients, families, and caregivers, including bereavement follow-up.
• Evaluate insurance coverage and connect families to community resources and benefits programs.
• Participate in IDG meetings and keep the team updated on psychosocial changes.
• Complete timely EMR documentation and support staff orientation.

Hospice Social Worker

LilyCare Hospice is a community-based hospice serving patients and families across New Jersey. We are hiring a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. We are looking for someone to join us part-time, with an opportunity to work full-time as well.

Duties and Responsibilities

• Serve as the Hospice Social Worker for patients and families on your caseload — assess psychosocial needs and build the social work portion of the individualized plan of care
• Provide counseling and support to patients, families, and caregivers, including anticipatory grief and bereavement follow-up
• Evaluate insurance coverage and financial need, and connect families to community resources and benefits programs
• Participate in IDG meetings and keep the interdisciplinary team current on psychosocial changes
• Complete timely, accurate EMR documentation and support volunteer and staff orientation

Compensation and Schedule

The Hospice Social Worker role pays $36 - $42/hour based on experience. Full-time, Monday–Friday day schedule. Territory is northern New Jersey, based out of Florham Park, NJ.

Skills and Qualifications

• Master of Social Work (MSW) from a program accredited by the Council on Social Work Education
• Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners
• One year of social work experience in a healthcare setting; hospice, home health, or geriatric experience preferred
• Valid driver's license and reliable transportation
