Job summary

LilyCare Hospice in Bedford, NH seeks a licensed Hospice Social Worker (LICSW) for field-based work with terminally ill patients and families. The role supports the interdisciplinary team, conducts psychosocial assessments, and helps navigate emotional and practical challenges of end-of-life care.

Requirements include an MSW from a CSWE-accredited program and active NH LICSW license, with strong communication and geriatric care experience. Full-time or part-time positions available.

Qualifications

• MSW from a CSWE-accredited program is required.
• Active NH LICSW license required.
• Minimum two years of geriatric care experience.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform psychosocial assessments and contribute to care planning.
• Provide counseling to patients, families, and caregivers.
• Evaluate insurance coverage and connect families to resources.
• Participate in IDT meetings and document visits.
• Support bereavement services and coordinate referrals.
• Maintain timely documentation and assist with training.
