Job summary

LilyCare Hospice is seeking a licensed Hospice Social Worker (LICSW) to join our interdisciplinary team in Bedford, New Hampshire. This field-based role visits patients at home and in facilities, delivering psychosocial support to patients and families, and coordinating with nurses, chaplains, aides, and physicians.

The ideal candidate holds an MSW from a CSWE-accredited program, an active NH LICSW license, at least two years of geriatric experience, and a commitment to compassionate end-of-life

Qualifications

• MSW from CSWE-accredited program is required.
• Active NH LICSW license is required.
• Minimum 2 years' experience working with a geriatric population.
• Hospice or palliative care experience is strongly preferred.
• Strong communication skills and ability to work with patients, families, and healthcare providers at all levels.
• Ability to navigate difficult family dynamics and grief with empathy and professionalism.
• Valid driver's license, reliable transportation, and ability to travel to patient homes and facilities.

Responsibilities

• Perform psychosocial assessments of patients and families and contribute to the care plan.
• Provide counseling and emotional support to patients, families, and caregivers through grief, crisis, or transition periods.
• Assess insurance coverage and financial needs; connect families with community resources.
• Participate in IDT meetings; communicate psychosocial updates and care plan changes.
• Make referrals to community agencies and coordinate services as needed.
