Job summary

LilyCare Hospice in New Jersey is seeking a part-time Hospice Social Worker to join our interdisciplinary team, serving patients and families across the state with compassionate psychosocial care.

Responsibilities include assessment, counseling, coordinating benefits, participating in IDG meetings, and accurate EMR documentation. The role offers a daytime schedule, northern New Jersey coverage from Florham Park, with potential to transition to full-time.

Qualifications

• MSW degree from a CSWE-accredited program.
• Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners.
• One year of social work experience in a healthcare setting; hospice/home health/geriatric preferred.

Responsibilities

• Serve as Hospice Social Worker for patients and families on your caseload; assess psychosocial needs and contribute to the care plan.
• Provide counseling and support to patients, families, and caregivers, including bereavement follow-up.
• Evaluate insurance coverage and connect families to community resources and benefits programs.
• Participate in IDG meetings and keep the interdisciplinary team updated on psychosocial changes.
• Complete timely EMR documentation and support volunteer and staff orientation.
