Job summary

LilyCare Hospice is hiring a part-time Hospice Social Worker to join our interdisciplinary team in northern New Jersey, based out of Florham Park. You will shape end-of-life care by assessing psychosocial needs and providing counseling to patients and families.

We offer benefits, flexible scheduling, and opportunities for full-time work. Applicants should have an MSW and NJ licensure, with hospice experience preferred and a valid driver's license.

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program.
• Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners.
• One year of social work experience in a healthcare setting; hospice/home health/geriatric experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Serve as Hospice Social Worker for patients and families on caseload; assess psychosocial needs and contribute to care plan.
• Provide counseling and support to patients, families, and caregivers including bereavement follow-up.
• Evaluate insurance coverage and connect families to community resources and benefits programs.
• Participate in IDG meetings and keep team updated on psychosocial changes.
• Complete timely EMR documentation and support staff orientation.

LilyCare Hospice is a community-based hospice serving patients and families across New Jersey. We are hiring a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. We are looking for someone to join us part-time, with an opportunity to work full‑time as well.

Duties and Responsibilities

• Serve as the Hospice Social Worker for patients and families on your caseload — assess psychosocial needs and build the social work portion of the individualized plan of care
• Provide counseling and support to patients, families, and caregivers, including anticipatory grief and bereavement follow‑up
• Evaluate insurance coverage and financial need, and connect families to community resources and benefits programs
• Participate in IDG meetings and keep the interdisciplinary team current on psychosocial changes
• Complete timely, accurate EMR documentation and support volunteer and staff orientation

Compensation and Schedule

The Hospice Social Worker role pays $36 - $42/hour based on experience. Full‑time, Monday–Friday day schedule. Territory is northern New Jersey, based out of Florham Park, NJ.
