Job summary

Luminos Hospice is seeking a Hospice Social Worker to join our part-time care team in Colorado. The role provides psychosocial assessments and counseling to patients and families under physician supervision, collaborating with the Interdisciplinary Group to coordinate holistic end-of-life care.

Requires MSW and current state social work licensure (LCSW/LMSW or equivalent) with at least 2 years in healthcare, preferably hospice.

Qualifications

• Master’s degree in social work (MSW) from an accredited program.
• Current state social work licensure (LCSW, LMSW, or equivalent) required.
• Minimum two years of experience in healthcare, preferably hospice/palliative care.
• Excellent interpersonal and communication skills; proficient in documentation.

Responsibilities

• Conduct psychosocial assessments to evaluate factors affecting the patient and family.
• Provide emotional support and counseling to patients and families.
• Coordinate care with the IDG/T to address all aspects of patient care.
• Assist families in understanding and managing grief and loss.
• Help families access community resources and services.
• Maintain documentation of social work services per hospice policies and regulatory requirements.
