Job summary

Luminos Hospice seeks a dedicated Hospice Social Worker to provide psychosocial assessments and counseling to patients and families under the attending physician. You will collaborate with the IDG/T to coordinate holistic, compassionate care tailored to patient needs.

The role requires an MSW, current social work licensure, and at least two years in healthcare, preferably hospice or palliative care. Strong interpersonal skills and empathy are essential.

Qualifications

• MSW from an accredited school of social work.
• Current state social work licensure (LCSW/LMSW or equivalent).
• Minimum two years in healthcare, hospice or palliative care preferred.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, social, and environmental factors affecting patients and families.
• Provide emotional support and counseling to patients and families coping with end-of-life issues.
• Coordinate care with the Interdisciplinary Group/Team (IDG/T) to address all aspects of care.
• Assist families in understanding grief, loss, and available community resources.
• Document social work services in accordance with hospice policies and regulatory requirements.

Requirements

Education

Master’s degree in social work (MSW) from an accredited school of social work.

Licensure

Experience

Minimum two (2) years of experience in a healthcare setting, preferably in hospice or palliative care.

Strong working knowledge of community resources and support services.
