Job summary

Luminos Hospice in Colorado seeks a Hospice Social Worker to provide psychosocial assessments and counseling to patients and families under the attending physician, collaborating with the IDG/T to coordinate holistic care.

The role requires MSW, state licensure (LCSW/LMSW or equivalent), 2 years healthcare experience, strong knowledge of community resources, and excellent interpersonal skills to support families through end-of-life processes.

Qualifications

• MSW from an accredited school of social work.
• Current state licensure as a social worker (LCSW, LMSW, or equivalent).
• Minimum two (2) years of experience in a healthcare setting, preferably hospice.
• Strong working knowledge of community resources and support services.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, social, and environmental factors affecting the patient and family.
• Provide emotional support and counseling to patients and families coping with end-of-life issues.
• Coordinate care with the IDG/T to address all aspects of patient care.
• Assist families in understanding and managing grief and loss.
• Help families access community resources and services.
• Maintain documentation of social work services in accordance with hospice policies and regulatory requirements.
