Job summary

Liberty Hospice seeks an experienced Nurse Practitioner for a part-time Palliative Care role covering Harnett and surrounding counties in North Carolina. Expect 2–3 days per week with compassionate, patient-centered care designed to relieve suffering.

Responsibilities include conducting medical visits, coordinating care with the Palliative Care team, and communicating with patients, families, and agency staff. Requires NC NP licensure and Adult/Family certification.

Qualifications

• Master’s degree in Nursing is required and active licensure in North Carolina.
• Certification as Adult or Family Nurse Practitioner is required.
• Hospice or palliative care experience is preferred.
• Knowledge of hospice care program structure and operations.

Responsibilities

• Implements palliative care program requirements.
• Completes medical visits with patients per guidelines and coordinates care with the Palliative Care team.
• Conducts face-to-face encounters to provide re-certification information to Medical Director.
• Provide clinical findings from visits to primary care physicians or Medical Director.
• Communicate with patients and families at all levels.
• Coordinate communication with all agency personnel.
