Job summary

Liberty Health in Jacksonville, North Carolina is seeking a Hospice & Palliative Care Nurse Practitioner for part time (2-3 days/week) to join Liberty Hospice. You will conduct visits, certify care needs, and coordinate with the Palliative Care team.

The role requires an NP with Adult or Family certification, nursing licensure in NC, and strong communication and organizational skills. Experience in hospice/palliative care is preferred; background checks and EOEs apply.

Qualifications

• Master’s degree in Nursing required, with current licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner.
• Experience in Hospice or Palliative Care nursing preferred.
• Knowledge of the framework, function, and organization of a hospice care program.
• Excellent communication and organization skills.

Responsibilities

• Implements palliative care program requirements.
• Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated.
• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients.
• Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director.
• Communicate with patients/families at all levels.
• Communicate with all agency personnel.
