Job summary

Liberty Health Supply in North Carolina seeks a part-time Hospice & Palliative Care Nurse Practitioner to join our compassionate team in Brunswick County. The role focuses on implementing palliative care program requirements and coordinating with the Palliative Care team.

The ideal candidate holds a Master’s in Nursing, certification as an Adult or Family NP, and has experience in hospice or palliative care. Strong communication and organizational skills are essential for this position.

Qualifications

• Master’s degree in Nursing required, with current licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner
• Experience in Hospice or Palliative Care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program.
• Excellent communication and organization skills.

Responsibilities

• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients.

• Communicate with patients/families at all levels.
• Communicate with all agency personnel.
