Job summary

Liberty Health in North Carolina is seeking a Hospice Registered Nurse to provide compassionate, patient-centered care to terminally ill individuals and support their families. This is a full-time position based in North Carolina with hospice care responsibilities.

You will work with physicians, patients, and caregivers to ensure comfort, dignity, and timely symptom management across home settings. Travel may be required, including evenings and weekends.

Qualifications

• Current RN licensure in state of practice, successful completion of a nursing education from an approved school of nursing with two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
• Knowledge of the home health and hospice care framework, the nurse’s role on the patient care team.
• Working knowledge of nursing principles per nursing practice act, agency policies and reimbursement guidelines.
• 12 hours of continuing education annually.
• CPR certification.
• Travel based on agency need and weekend/evening/holiday call assignments.

Responsibilities

• Apply a working knowledge of nursing theories and concepts.
• Responsible for care and documentation of care which meets reimbursement guidelines.
• Coordinate physical care of the patient with family, patient, physician and Hospice team.
• Communicate with patients/families at all levels.
• Communicate with all agency personnel.
• Attend and participate in IDT meetings and quality review as scheduled by agency.
