Job summary

Sangre de Cristo Community Care - Hospice is seeking a compassionate Part Time Certified Nursing Assistant (CNA) to provide hands-on hospice and home health care in Colorado.

In this role, you will assist patients with daily activities, monitor conditions, and collaborate with an interdisciplinary team to deliver dignified, patient-centered care in home or residential settings. Scheduling is flexible to meet patient and organizational needs.

Qualifications

• Colorado CNA certification is required.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team.
• Compassionate, empathetic, and patient-focused.
• Experience in hospice care preferred but not required.

Responsibilities

• Provide hands-on hospice/home health care in patients' homes or facilities.
• Assist with activities of daily living: bathing, grooming, dressing, toileting.
• Observe and report changes to the interdisciplinary team.
• Offer emotional support and companionship to patients and families.
• Maintain a clean, safe, and comfortable environment.
• Collaborate with nurses, doctors, social workers for coordinated care.
