Job summary

Evolving Solution Services is seeking a Certified Nursing Assistant for a part-time role in Cortez, CO. The CNA will provide direct patient care and assist with transportation as needed, following the Plan of Care developed with the Interdisciplinary Team.

Under the supervision of a registered nurse, you will assist with activities of daily living, monitor patient condition, and document care accurately. Colorado CNA license and at least two years of patient care experience are required.

Qualifications

• Minimum of two years direct patient care experience in hospital, community or home setting.
• Documentation of successful completion of a patient care training program at least 40 hours in length.
• Must be licensed as a CNA in the State of Colorado.

Responsibilities

• Provide essential personal care to the patient under RN supervision.
• Assist with activities of daily living and transport as needed.
• Maintain required competency and complete documentation prior to timesheets.
